Job Scope:
Responsible for the management and expansion of services in Kenneth Young Center's Housing First supportive housing programs, including scattered site PSH (Permanent Supportive Housing), multiple PSH apartment buildings in the Northwest Suburbs, a No-Income PSH program, and a Transitional Living Program (TLP).
Ensure grant deliverables are met for multiple grants and complete required tracking and reporting to State and PSH partners.
Provide clinical and administrative oversight to multiple teams providing a variety of comprehensive mental health and community support services, case management and supportive housing services.
Oversee KYC's SOAR (SSI/SSDI Outreach Access and Recovery) services to assist clients in various programs to obtain benefits.
Proof of COVID-19 vaccination required.

Responsibilities include:
Oversee overall operations for KYC's Housing First Supportive Housing programming; including scattered site PSH (Permanent Supportive Housing), multiple PSH apartment buildings in the Northwest Suburbs, a No-Income PSH program, and a Transitional Living Program (TLP) Oversee and coordinate TLP admissions, discharges, and services within the Transitional Living Program Coordinate communication, service provision, and continue to build relationships with 4 area PSH partnerships; act as liaison with on-site property managers and supervisor Coordinate with UP Development and other local property managers regarding services provided at PSH locations Coordinate with Burton Group and oversee services provided at PSH locations in Kane County Accept and coordinate referrals from UP Development and arrange for mobile intake and assessment to determine appropriateness and recommendations Ensure outreach and brief case management services are available and provided to all tenants at 2 PSH buildings (Philhaven, Myers Place) Work cooperatively with TLP Building Management company, PSH property managers, and scattered site property managers to troubleshoot tenant/landlord issues and ensure continued tenancy for clients and KYC Ensure that all services and clinical activities provided by all team members are compliant with standards, policies and procedures of the organization and clinical best practices; integrating all facets of clients' lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use issues, spiritual, community-integration, and housing needs Monitor, oversee, and approve all Housing First No-Income (NIH) expenditures to ensure meeting grant requirements, KYC policy, and identify and encourage alternatives to ensure fiscal responsibility Ensure timely and accurate completion of daily data reporting, weekly, monthly, and quarterly reports with timely submission to DHS/DMH Monitor progress and ensure meeting all grant deliverables and targets for Housing First No-Income (NIH) Oversee referrals for the Housing First No-Income program and SOAR (Benefits specialists) services Coordinate with Manager of ACT/CST, Manager of Front Door, and Manager of Data and Compliance to ensure all team members are working together effectively in care provision and grant reporting Act as a liaison between agency departments (PSR, SUPR, medication monitoring, Older Adult, Intake, Employment, Supportive Housing, etc) to ensure service coordination and continuity of care Monitor program staffing needs and participate in hiring and orientation of new staff, corrective action, and termination as necessary Individual Performance Review and approval all Housing First No-Income stipend requests and evaluate the client's active participation in care and employment search/benefits application, following rules of lease, and behaviors meeting Housing First NIH program requirements Accurately complete clinical documentation and service reporting in a timely manner meeting KYC policy (currently 5 days for progress notes, 14 days for IM-CANS) Attend monthly blended management meetings with UP Development property managers and KYC clinical staff at Philhaven and Myers Place Complete at minimum 2 IM-CANS reassessments per month for unassigned PSH residents to ensure continuity of service and continued clinical expertise Provide direct service outreach and in-home clinical mental health services to small caseload of PSH residents; at least 2-5 visits per week Provide weekly and ad-hoc clinical and administrative supervision of direct reports (Supportive Housing Supervisor, Supervisor of Housing First No-Income Housing) Participate in department budget process, assertively participate in monthly finance meetings, monitor progress towards revenue goals and expenditures Actively monitor all team member's productivity and grant milestone progress and provide coaching, feedback, and other performance improvement steps as needed to ensure individual and team revenue goals are met Participation in program planning, service coordination, practice development and quality assurance as a member of PSC (program services committee) and CQI (continuous quality improvement) committee Participate in scheduled department and division manager meetings Participate in orientations, all staff meetings and training/ development Interpret and communicate, at the team member and service delivery level, Center policies and procedures Field calls from clients and PSH partners with concerns/complaints, and provide effective mediation and customer service Submit, in a timely and accurate fashion, schedules, timesheets, mileage and other reports required by the Center's policies and procedures Review supervisees clinical work including IM-CANS assessments and re-assessments, Service Plans, Rule 140 IATP (integrated assessment and treatment plans), LOCUS and DLA, and other comprehensive mental health services Complete performance evaluations for all supervisees building on strengths and identifying ongoing growth/training needs Participate in clinical/administrative supervision; seek individual consultation with supervisor when needed.
Participate in rotating 24/7 backup on-call (licensed staff) and coverage for on-call shifts as needed Provide in-office support as manager on-site multiple times per week Other duties as assigned Education and Work Experience Masters Degree from an accredited college or university in social work, psychology or a related discipline; meets IL Rule 140 QMHP (Qualified Mental Health Professional) designation Preferred LCPC/LCSW/LMFT in IL that meets Rule 140 LPHA designation Preferred at least 4 years of experience working with individuals with serious mental illness, co-occuring substance use, and/or persons experiencing chronic homelessness Preferred at least 2 years working in non-traditional treatment environments i.

in-home, residential, hospital, nursing home, community settings Experience in program management and/or development of mental health programming At least 2 years of demonstrated ability to provide clinical and administrative supervision IM-CANS certified or obtain within 30 days Basic technology and computer proficiency:
