Northwest suburbs; primarily 20 minute radius around Elk Grove Village/Mt Prospect Site:
Primarily community-based with in-office expectations, limited amount of weekly remote/work from home Compensation & Benefits Starting $54,000-$58,000 with increases for bilingual and LSW/LPC or LPHA license, and CRSS/CADC certifications 37.
5 hour workweek; ability to work schedule that includes days and weekends Robust benefits package & starting 4 weeks PTO (additional details below) Job Scope Provides in-person therapy, supportive counseling, community support, case management, and crisis stabilization services to adults with serious mental illness living in the community Complete comprehensive mental health assessments, crisis plans, and recommendations/integrated treatment plans using IM-CANS tool (training/certification provided) Work with team members to support a shared caseload of clients (assigned to either Assertive Community Treatment Team or Community Support Team) Promote stabilization thru medication monitoring, psychoeducation, and skill development to support individuals to engage fully in their lives Participate in team services to assist individuals to establish access to benefits and entitlements, understand and participate in activities to manage medical conditions, and minimize any negative impacts of substance use Provide assertive engagement and utilize creative techniques to support individuals to build natural systems of support thru recreational, spiritual, educational, vocational, and volunteer activities Provide support to individuals who are unhoused or residing in temporary housing to locate permanent housing Meet with adults in their homes and other community spaces; ability to drive clients as part of treatment Meet productivity target estimated at 45-55% and complete daily progress notes Routinely coordinate with other treatment teams to support clients transitioning to the community thru Williams/Colbert Transition Team and Front Door Diversion Participate in rotating on-call schedule of primarily phone support to ensure 24/7 coverage (additional stipend provided) Commitment to Housing First, Harm Reduction, Dignity of Risk, Self-Determination and Recovery Model principles Education/Experience Preferred Master's Degree in social work, psychology or related discipline.
Required Bachelor's Degree in related field with mental health experience.
Preferred experience working with adults with serious mental illness Preferred exposure to working in non-traditional treatment environments (i.
e.
in home, residential and supportive housing, hospital, and/or nursing facility) Basic technology, smartphone and computer proficiency; knowledge of web-based team communication tools (ex.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
